Please print and fill out attached attendance form by Oct 5th. We will be using a paper form at least through first semester.

Our effort to change the monthly attendance tracking, which you provide to the school, from a paper form to an on-line electronic system will be delayed at least until the second semester.

This means that we need you to complete your student’s attendance for Month 2 on the paper tracking form.
You may obtain a copy of this form and the following months from our website at http://www.mountainhomecharter.org/attendance-forms.html or http://www.glacierhighcharter.org/attendance.html or at our school office.

Please note - 

• Make sure to sign each line in blue or black ink and mark subjects covered.
• A child is not considered absent if at least one subject was covered for that day.
• If you mark absent on the form it is important to write in the reason for the absence (such as sick, ill, doctors appointment, etc) otherwise this absence is considered un-excused. If you have 3 or more un-exused absences in the year your child is considered truant by California Law. 

This completed and signed attendance form will be due to our office on Friday, October 5th by 2pm.
