Student Enrollment Requirements:

1. Completed and signed Student Registration packet
2. Emergency Procedure Card
3. Right to Privacy
4. Request for Cumulative Records
5. Student Internet Use Policy and Agreement
6. Birth Certificate
7. Immunization records + Tdap booster (or signed waiver available from secretary)
8. Proof of TB test (or signed waiver)
9. Proof of residence (copy of utility bill, etc.)
